Form templates and submissions
Form templates
Form templates define what information your agent collects during calls. Access Form Templates from the Build section of the sidebar (visible when at least one agent uses forms).
Creating a template
Click New Form.
Add fields: text, phone, email, and other field types.
Set up conditional logic (if/then rules) for dynamic forms.
Save the template.
Template statuses
Draft - Being designed, not assigned to agents
Published - Available for agents to use
Archived - No longer active
Assigning to agents
Assign a form template to one or more agents with an enable/disable toggle. When a caller provides information matching the form fields, the agent captures it.
Form submissions
View all submitted forms at Form Submissions in the sidebar.
Submission list
Contact/patient name
Form template used
Date submitted
Status (Submitted, Amended, Tombstoned)
Submission detail
Click any submission to see:
All captured field data
Option to Amend (edit/add missing fields)
Export to PDF
Tombstone option (redact PII while preserving audit record, for HIPAA compliance)
Bulk export
Export submissions to CSV from the submissions list.
Auto-fill
When your agent calls a known contact, form fields can be pre-filled with existing contact data.
